2. Be sure to complete all of sections B regarding information pertinent to the joint owner, if there is a joint account owner on your account who needs access to the account through myMAX Online Branch.
3. Sign the application. You may mail the application to MAX, Attn: Telephone Service Center, PO Box 244040, Montgomery, AL 36124, or bring it to any of our convenient MAX branches.
