To "Become a MAX Member" simply print and complete a membership application. To join there is a one-time $1 membership fee and a minimum deposit of $5, which represents one share of the Credit Union.

Primary Owner
• All fields required
• Provide both Home and Work phone numbers for follow-up contact purposes

ID Type and Number
• Field required

Joint Owner
• Should be completed if there is to be a joint owner
• Fill in all fields that apply to the joint owner
• If the joint owner’s information is the same as the primary owner, just enter “same” in the boxes

ID Type and Number

Relative or Joint Owner not living with you
• Field required

Mother’s Maiden Name
• This field provides an added security to your account

Share Draft Account Plan
• If you are interested in opening a checking account, specify which account would better suit your need
